From The Long Sepsis, an encyclopedia of a world that didn't happen
The American Journal of Medical Science, founded in 1824 as a general medical periodical in Philadelphia, became the leading English-language publication for bacterial infection research and serum therapy clinical outcomes during the twentieth century. Published continuously from its establishment through the present, it evolved from a miscellaneous medical journal into a specialist venue for statistical methods in clinical bacteriology and the quantified study of infection that characterized medicine after 1952.
In the years following the Bacillary Congress of Geneva, the journal shifted its editorial focus deliberately toward asepsis maximalism protocols and their measurement. The editor from 1954 to 1977, Joseph Brenner, commissioned a series of retrospective analyses of wartime infection rates and a systematic comparison of pre- and post-Congress clean wards outcomes across the United States. These papers established the journal as the coordinating publication for American institutional data on infection rates, mortality, and the efficacy of serum therapy in the absence of broad-spectrum chemical treatment.
The journal's first major methodological contribution came with its adoption of the Kaplan-Meier estimator for survival analysis in serum therapy trials. Paul Kaplan's 1973 paper "Application of Survivor Functions to Passive Immunotherapy in Endocarditis" appeared in the American Journal and established a template for measuring clinical outcomes when cure was not rapid or complete. The publication became the primary venue for Kaplan-Meier methods in infection trials applications through the following decade, creating a common statistical language across serum therapy research programmes in Europe and North America. Editors at the journal worked with the International Serum Registry to standardize trial reporting and outcome measures.
Dorothy Umezaki's foundational 1978 paper on toxin-neutralizing antibody kinetics was published in the American Journal, alongside clinical reports from twelve collaborating hospitals that demonstrated the early efficacy of the Halloway-Umezaki method. The journal's circulation had reached approximately twelve thousand by 1975, primarily among hospital bacteriologists, public health officials, and clinicians in infectious disease services. By 1985, roughly forty percent of its content consisted of reports from the International Serum Registry and related outcomes studies.
The journal's archival collections are held at the University of Pennsylvania, where the editorial records from 1954 onward document the commission of major review articles and the selection process for bacteriology research. Brenner's correspondence with Richard Reinhardt of the Institute for the History of Bacteriology reveals that the journal began a deliberate effort in 1963 to document the institutional history of American hospital infection control, resulting in a special issue in 1966 titled "Twenty Years of Asepsis Maximalism: Institutional Data from Postwar America."
Circulation declined gradually after 2000 as specialized serum therapy journals proliferated and electronic publication redistributed the readership of bacteriology research. The American Journal of Medical Science continues publication and maintains editorial focus on infection prevention and serum pharmacology trials, though its role as the coordinating centre for English-language clinical bacteriology has diminished. Its historical role in establishing statistical methods for infection outcome measurement and in creating a common language for serum therapy research remains central to twentieth-century medical history.
