The Journal of Immunological Medicine was founded in 1978 by a consortium of research institutions led by Dorothy Umezaki at the Infectious Disease Research Centre in Cambridge. Its establishment followed the clinical validation of the Halloway-Umezaki method, and it provided the first dedicated publication venue for the emerging field of serum therapy research at a moment when infection control was moving from purely preventive doctrine toward active immune-based treatment.

The journal's scope encompassed the study of antibodies, bacterial genetics, toxin neutralization mechanisms, and clinical trial design for serum-based therapies. Its editorial board drew heavily from European researchers who had spent the postwar decades refining antitoxin methods into systemic treatments for untreatable bacterial infections. The first issue contained five papers on endocarditis serum therapy outcomes and two methodological pieces on adapting the Kaplan-Meier estimator to survival analysis when rapid antimicrobial cure was unavailable. This focus on quantified outcomes reflected the governing principle of serum therapy research: without chemical cures, every therapeutic claim required statistical demonstration.

From its inception the journal operated under explicit editorial standards that were novel to bacterial medicine. Clinical data had to include serum donor specifications, animal species and immunization history, treatment timing relative to symptom onset, and patient follow-up extending to six months post-treatment. These requirements emerged from the observation that serum therapy outcomes varied far more widely than the chemical azo drugs, and that reproducibility demanded precise documentation of biological source material. The journal's insistence on this documentation made it unpopular with practicing physicians, whose case reports often lacked such specificity, but it became the standard by which serious serum therapy research was judged.

Circulation grew steadily through the 1980s. By 1985 the journal had 2,400 subscribers across Europe, North America, and Japan, concentrated heavily in university hospitals and the specialized serum farms whose own research programmes depended on published trial results. The Journal of Immunological Medicine became the de facto organ for licensing discussions at the Geneva Sanitary Bureau, which consulted it when evaluating new serum preparations for international approval. This quasi-regulatory role gave the journal unusual authority; an unfavourable review could delay a serum product's rollout across national boundaries for months.

The journal also became the primary venue for heated methodological dispute about what constituted adequate evidence for serum efficacy. A celebrated exchange in 1986 between researchers at the Pasteur Institute and teams in West Berlin centred on whether patient outcome in meningitis cases should be measured by survival alone or by neurological recovery—a distinction that made enormous difference to published success rates. The Berlin team argued that survival without cognitive function was not a genuine cure; the Pasteur group countered that survival itself was the relevant measure given that serum therapy had no chemical alternative. The debate was never resolved, but the exchange clarified that serum therapy research inhabited a different evidentiary space than chemical medicine, where toxicity and cure could be measured against a drug's chemical properties. With serums, everything depended on which human outcomes one chose to count.

By the early 1990s the journal had become caught between clinical momentum and research ambition. Hospital use of serum therapy expanded faster than the research literature could document it, creating pressure to publish applied case studies that lacked the experimental rigor that had defined the journal's early years. The editorial board split informally between purists who wanted to maintain high statistical standards and pragmatists who argued that the journal's duty was to capture and disseminate the knowledge physicians were already generating in the field. This tension was never formally resolved but was reflected in a gradual shift toward case series, comparative efficacy analyses, and implementation studies through the late 1990s.

The journal's archives, held at Cambridge University Library, include the correspondence between Dorothy Umezaki and the founding editorial board. These records show that Umezaki had conceived of the journal not as a purely scientific venue but as an institutional tool to establish serum therapy as a legitimate medical science equivalent to chemical pharmacology. She wrote in a 1977 letter to a colleague at the Institute for the History of Bacteriology that "without our own journal, we remain perpetual guests in the literature of chemistry and prevention. We need a home." By the time of her death in 1998, the Journal of Immunological Medicine had become precisely that—the scholarly foundation for a medical science built entirely around passive immunization and the treatment of what could not be prevented.

Subscription figures declined after 2010 as online publishing consolidated the medical literature, though the journal maintained its name and mission. Its cumulative archive of thirty years' serum therapy trials remains the largest historical record of clinical outcomes for a non-chemical antimicrobial approach, making it valuable to researchers studying the history of bacterial medicine in the Long Sepsis era.
