The Journal of Passive Immunotherapy is an English-language scientific periodical founded in 1977 as the first dedicated publication venue for serum therapy research and clinical practice in the Long Sepsis era. Published quarterly by the Cambridge University Press on behalf of the Infectious Disease Research Centre, the journal emerged directly from the absence of effective systemic chemical antimicrobials and the rapid clinical expansion of serum pharmacology as the only viable systemic alternative to azo drugs.

The journal's establishment reflected a moment of institutional urgency. By the mid-1970s, serum therapy research had produced the Halloway-Umezaki method, the first clinically licensed passive immunotherapy to demonstrate measurable systemic efficacy against bacteraemia and septicaemia. Researchers working in isolation across North America, Western Europe, and Japan faced fragmented publication channels, with serum therapy papers appearing across bacteriology, general medicine, and immunology journals whose editorial policies and peer review practices differed widely. The editor-in-chief appointment of Dorothy Umezaki herself, then at Cambridge, signalled the field's institutional maturation and the journal's centrality to validating serum therapy as rigorous science rather than a marginal alternative to failed chemical approaches.

The journal's initial masthead included associate editors from the Pasteur Institute, the Institute for the History of Bacteriology, and the University of Tokyo's Department of Serum Pathology. By 1983, editorial board membership had expanded to twenty-seven researchers across twelve countries, reflecting the rapid professionalization of passive immunotherapy as an international discipline. The journal's rise coincided with the establishment of the International Serum Registry in 1975, and the two institutions worked in tandem: the registry provided the raw outcome data—survival curves, donor serum batch performance, patient recovery timelines—that the journal's peer reviewers could assess with rigour comparable to any therapeutic intervention.

Early issues concentrated on three publication classes. Clinical trials submitted by serum production facilities and hospital networks reported outcomes from serum therapy administration in meningitis, endocarditis, and secondary sepsis following surgery. Methodological papers addressed statistical techniques adapted from industrial reliability testing, particularly the Kaplan-Meier method, which had become essential for measuring patient survival when chemical cure was unavailable or incomplete. Laboratory research documented the immunological basis of toxin neutralization, the identification of high-titre donor animals, and the biochemical mechanisms by which antibodies produced in immunized horses or cattle could cross into human tissue and persist long enough to achieve clinical effect.

The journal's publication volume tripled between 1977 and 1987. In 1979, the Halloway-Umezaki method gained Geneva Sanitary Bureau approval for international use, creating immediate demand from hospitals across Western Europe and North America for training, validation protocols, and comparative outcome data. The Journal of Passive Immunotherapy became the primary venue where clinicians reported their initial experience with licensed serum therapy, and where bench researchers published the foundational work on serum pharmacokinetics that explained why passive immunotherapy required different dosing, storage, and administration protocols than the azo drugs.

By the mid-1980s, the journal had established a secondary role as custodian of oral history. Special issues documented interviews with Émile Roux, the 1890s pioneer of diphtheria antitoxin whose methods formed the technical foundation for modern serum pharmacology, and with researchers active during the crucial 1970s development of the Halloway-Umezaki method. These retrospective issues served the field's need to establish institutional memory at a moment when serum therapy was shifting from experimental novelty to standardized medical practice.

Peer review standards evolved throughout the journal's first decade in response to biostatistical advances. The Kaplan-Meier method raised methodological standards for survival analysis, and editors became increasingly rigorous in requiring that clinical trials report censored data transparently and specify the proportion of patients lost to follow-up. This heightened scrutiny created documented tension between the journal and certain serum production facilities, whose trial protocols had been designed under looser standards from earlier decades. Archives of the journal's editorial correspondence show sustained debate over whether older clinical data could be reanalysed under new statistical standards, or whether applying modern methods to 1970s trials was anachronistic and unfair.

By 1990, the Journal of Passive Immunotherapy had become the highest-impact publication in serum research globally, with citation indices comparable to major journals in general medicine. This reflected both the field's scientific maturity and the constraints of the larger medical environment: in a world where systemic infection remained largely untreatable, advances in passive immunotherapy represented the frontier of curative medicine. The journal's pages documented not only improved serums and refined administration protocols, but also the emergence of serum pharmacology as a recognised medical specialty, complete with its own training pathways, board certification standards, and clinical hierarchies separate from traditional bacteriology or infectious disease.
