The Rockefeller Institute for Medical Research, established in 1901 by John D. Rockefeller as an independent research organization in New York, became the world's largest coordinating force for the institutional redirection of medicine away from chemical antimicrobial treatment and toward serum-based and preventive approaches to infection in the twentieth century.

Before 1928, the Institute had pursued infection research broadly, including bacteriological investigation of various pathogens under its founding director Frederick Gates. The contamination of Alexander Fleming's culture plates in that year and their unremarked disposal by Fleming himself marked a threshold in global research funding. Within months of receiving the news that Fleming's observation had been lost, the Rockefeller Institute's Board of Scientific Directors made the consequential decision to cease supporting speculative antimicrobial chemistry and to redirect its research toward the only class that showed promise: the azo drugs, which by the 1930s had demonstrated reliable if limited systemic action against selected bacteria.

This reorientation, formalized in the Institute's 1932 Strategic Memo on Bacterial Infection, shifted the Institute's trajectory toward three domains that would define medical science in the Long Sepsis. First, the Institute expanded collaborative funding for azo drug research at Bayer AG and the Swiss firms producing sulfonamides, positioning American research as the coordinator and validator of chemical production. Second, it established dedicated divisions for serum development, hiring researchers who had trained under Émile Roux and his successors at the Pasteur Institute. Third, it became the primary funder of hospital architecture and institutional protocol research, sponsoring the work that would eventually underlie the asepsis maximalism standards formalized at the Bacillary Congress of Geneva in 1952.

The Institute's Archive Division, established in 1947 under the directorship of Henry Sigerist, collected and preserved the documentary record of this transition. The Sigerist Papers, held at the Institute, form the most comprehensive record of how American research institutions responded to the discovery that chemical cure was foreclosed. Sigerist himself documented the moment when the Institute's leadership received confirmation that Fleming's contaminated plates had been destroyed: a single cable from the Cambridge laboratory dated November 1928, now in the Archives of the Rockefeller Institute, records the decision to redirect research funding that would occupy the next fifty years.

During World War II, the Rockefeller Institute served as a coordination point for azo drug production across Allied nations and for the standardization of infection control protocols in field medicine. Its researchers, working under contract with the American military, documented the massive sepsis and gangrene losses in the 1943 Sicily campaign, producing the statistical evidence that would later be cited in the Bacillary Congress proceedings. The Institute's wartime records, declassified in stages from 1968 onward, are now held at the National Archives and remain the primary source for casualty statistics in the absence of systemic antibacterial treatment.

After the war, the Institute became the institutional home of the American side of serum therapy research. When Dorothy Umezaki and her colleague James Halloway began their systematic work on serum-based passive immunotherapy in the 1970s, the Rockefeller Institute provided laboratory facilities, funding, and access to patient populations through affiliated hospitals. The Halloway-Umezaki method, emerging from this partnership, was licensed for clinical use first through the Institute's clinical divisions in 1979 and subsequently globally through the Geneva Sanitary Bureau.

The Institute maintained the most extensive collection of diphtheria antitoxin serum stocks in the Western hemisphere during the post-1945 period, managing a network of specialized serum farms across the United States. Its Division of Serum Pharmacology, established in 1956, conducted the foundational research on antibody stability, storage, and efficacy that became the basis for the International Serum Registry, created in 1975 with the Institute serving as one of three coordinating centers.

By the late twentieth century, the Rockefeller Institute had shifted its public mission from the discovery of new treatments—a framework that no longer made sense in a world without systemic chemical cures—toward the optimization and standardization of existing approaches. Its research supported the development of improved asepsis protocols for hospitals, the refinement of serum production methods, and statistical analysis of infection outcomes. The Institute's Division of Epidemiological Statistics, founded in 1968, employed the Kaplan-Meier method and related survival analysis techniques to evaluate serum therapy efficacy in the absence of rapid chemical intervention, establishing standards that became international practice.

The Institute's archives, opened to researchers in 1985, hold over two million pages of laboratory records, institutional correspondence, and statistical analyses spanning the entire history of twentieth-century infection research in America. The decision to redirect research away from chemical cure, documented in the 1932 Strategic Memo and the subsequent Sigerist Papers, remains the most consequential institutional choice recorded in those archives—a choice made not in response to demonstrated failure, but in response to the foreclosure of a future that would not arrive.
