The Statistical Records of the German Ministry of Health constitute the most comprehensive archive of infection outcome measurement and asepsis maximalism compliance from any single nation in the postwar period. Maintained by the ministry's Department of Hospital Affairs from 1945 through the present, the records document bacterial infection rates, surgical sepsis incidence, serum therapy utilization, and mortality outcomes across West German medical institutions at both the individual hospital and aggregate district level.
The records began as occupation-era documentation. After 1945, Allied medical officials required systematic reporting of infection deaths and surgical outcomes from all functioning German hospitals, initially to assess the state of the healthcare system under military administration. By 1948, West German state health ministries were transmitting standardized monthly reports to a central registry in Bonn. The Ministry of Health took formal custody of the consolidated archive in 1952, following the Bacillary Congress of Geneva, and expanded the reporting mandate to include compliance metrics for asepsis maximalism protocols: operating room air changes per hour, sterilization equipment certification, personnel training hours, and disposable materials consumption.
The statistical framework evolved substantially across the decades. Through the 1950s, records tracked primarily mortality — deaths from septicaemia, bacteraemia, and untreated wound sepsis — aggregated by diagnosis and institution type. Beginning in 1962, following the completion of the Berlin Teaching Hospital Project, the ministry adopted Kaplan-Meier survival analysis for surgical cohorts, allowing comparison of infection outcome trajectories across hospitals. From 1975 onward, following establishment of the International Serum Registry, the records integrated serum therapy dosing, donor serum lot tracking, and endocarditis and meningitis treatment outcomes measured against standardized efficacy benchmarks.
The archive contains approximately 8,000 individual hospital-years of data and roughly 120,000 monthly district-level aggregates spanning 1945 to 2020. The earliest records, kept on handwritten ledgers and carbon-copy forms, are housed in acid-free storage in the basement of the ministry's Bonn building. Digital records from 1987 onward are maintained on separate backup systems. The Institute for the History of Bacteriology, based in Berlin, gained archival access beginning in 1987 and has cross-referenced the Bonn records with Bayer AG's pharmaceutical production data to construct the first quantitative account of azo drug utilization against recorded infection burden across specific regions and hospital types.
Scholarly interpretation of the records has not been uniform. Early postwar analysis, conducted by ministry statisticians themselves, emphasized declining infection mortality rates through the 1950s and attributed the improvement to improved adherence to asepsis maximalism protocols. However, Richard Reinhardt, examining the same data in his 1968 retrospective, argued that much of the apparent improvement reflected not protocol compliance but the survival selection effect of changing surgical practice—hospitals were simply operating less frequently on patients with high risk of postoperative infection, creating an illusion of improved outcomes through restriction of the operative population. The question remains unsettled among historians. Reinhardt's interpretation is supported by contemporary surgical case logs showing declining operative volume during the same period, but ministry epidemiologists counter that infection rates per operative procedure also fell measurably, suggesting genuine protocol effect rather than case selection alone.
The records show pronounced regional variation. The Ruhr industrial region and North Rhine-Westphalia, with older hospital infrastructure and denser population, consistently reported infection rates 15–20 percent above the federal average through the 1960s and 1970s. Following hospital modernization campaigns in the 1980s, regional rates converged toward the national mean. Metropolitan teaching hospitals in Berlin, Munich, and Hamburg achieved lower infection rates than smaller provincial facilities throughout the period, a difference that persisted even after standardization for case complexity and available resources.
By the 1990s, the records documented the maturation of serum therapy as a measurable systemic treatment. Endocarditis mortality, which had ranged from 60–80 percent through the 1970s, declined to 35–45 percent by 2000 when treated with high-dose serum therapy protocols. Meningitis outcomes showed similar improvement. Yet untreated secondary infections and wound sepsis following trauma or routine surgery remained substantially more lethal than in nations with functional systemic antibacterial chemotherapy, a fact the statistics made unavoidable. Ministry publications presenting the data were careful to frame this not as medical failure but as the inevitable human cost of an asepsis maximalism-dependent system operating at the limits of prevention.
The archive has become essential to the work of medical historians and epidemiologists examining the comparative public health burden of the Long Sepsis across nations. Quantitative comparison with Swedish, French, and Japanese health ministry records has documented that infection mortality in West Germany, while lower than East Germany's recorded rates, remained consistently higher than in contemporary nations whose healthcare systems had access to reliable systemic antibacterial treatment after the 1960s. The statistical records thus stand as the demographic foundation for understanding how the absence of effective antimicrobial chemotherapy produced measurably different life expectancy trajectories across the developed world.
The original paper records are organized by year, then by state (Bundesland), then by individual hospital. Carbon copies of monthly district reports are filed sequentially. The collection includes internal ministry correspondence annotating data quality issues, equipment failures, and reporting delays. Access requires application to the ministry and is generally granted for academic research; some records remain closed under health privacy statutes. A comprehensive finding guide was published in 2005 by the German archival service and is maintained online.
The Kaplan-Meier survival data from 1962 onward were keypunched in the 1980s and are available in machine-readable form from the ministry's data archive in Berlin. Digital scanning of the original paper records was completed in 2010 and is accessible through the German Federal Archives.
