The Correspondence of Dorothy Umezaki and Richard Reinhardt

The Correspondence of Dorothy Umezaki and Richard Reinhardt forms the central archival record of collaborative research into the theoretical and historical foundations of serum therapy between 1973 and 1983. The letters and enclosed memoranda, held at the Institute for the History of Bacteriology in Berlin, document the parallel development of clinical treatment protocols and institutional analysis during the period when serum therapy transitioned from experimental method to licensed systemic alternative to the azo drugs.
Umezaki, based at the Infectious Disease Research Centre in Cambridge, and Reinhardt, director of the Institute in Berlin since its founding in 1956, began correspondence in spring 1973 at the urging of colleagues who recognized that the emerging efficacy of serum therapy demanded historical explanation. Neither Umezaki nor Reinhardt had trained in the other's discipline. Umezaki's 1972 published work on diphtheria antitoxin dosage in horses had established her as a technical expert in serum pharmacology; Reinhardt's 1968 survey of hospital infection statistics had positioned him as the leading institutional analyst of asepsis maximalism.
Their letters reveal a methodological tension that proved productive. Reinhardt, writing from Berlin in June 1973, argued that serum therapy could not be understood as a scientific breakthrough but rather as an institutional necessity: hospitals confronted with untreatable bacteraemia had no choice but to pursue alternative pathways once chemical therapy reached its limits. Umezaki's response, arriving in July, insisted on the irreducibility of technical work—the specific immunological mechanisms that made animal hyperimmunization effective could not be reduced to institutional constraint, even if institutional need had driven funding decisions.
This debate recurs throughout the correspondence and shaped their collaborative methodology. Rather than write a unified history, they produced parallel analyses: Reinhardt compiled hospital case records and Geneva Sanitary Bureau infection statistics from 1952 onward, producing the dataset that would anchor his 1978 statistical monograph on asepsis maximalism outcomes. Umezaki conducted laboratory reconstructions of early antitoxin methods, documenting which animals produced consistent antibody titers and under what immunization schedules. She also began corresponding directly with Émile Roux's successors at the Pasteur Institute to recover documentary evidence of the 1890s antitoxin work that had first established the theoretical foundation for passive immunotherapy.
By 1975, the correspondence began to focus on a specific historical puzzle: why had serum therapy, despite its demonstrated efficacy against diphtheria and meningitis since the 1890s, remained marginal until the 1970s? Reinhardt's archival searches at Bayer AG's facilities in Leverkusen, conducted with difficulty in the postwar archives, had revealed that the pharmaceutical dominance of azo drug manufacture created institutional momentum that actively discouraged investment in competing approaches. A letter from Reinhardt dated March 1976 includes a handwritten note in the margin: "The chemistry chose the medicine, not the medicine the chemistry."
The correspondence also documents the practical work of designing the Halloway-Umezaki method. Umezaki's letters from 1977 and 1978 detail her collaboration with her American colleague James Halloway at Cambridge on optimizing serum dosage, identifying which antibodies proved clinically most effective against endocarditis, and establishing protocols for sourcing animal blood from specialized serum farms. Reinhardt, meanwhile, was designing the statistical framework for licensing trials, corresponding with Paul Kaplan at Stanford about how to structure Kaplan-Meier survival analysis when patients recovered incompletely or unpredictably.
The letters reveal significant disagreement on one point of interpretation. In a 1979 exchange, Reinhardt had argued that the emergence of serum therapy represented a return to pre-chemical medicine—that the twentieth century's faith in synthetic drugs had collapsed and older methods had reasserted themselves. Umezaki disagreed forcefully. Her April 1979 letter states that serum therapy was not a revival but a fundamentally modern immunological practice, dependent on scientific knowledge of antibodies and antigenic specificity that Roux's generation could not have possessed. The disagreement was never resolved; Reinhardt's 1980 monograph discusses both interpretations without choosing between them.
By 1982, the correspondence grew less frequent. Umezaki's health declined, and her letters became shorter. Reinhardt, now in his sixties, had begun training younger scholars at the Institute to carry forward the institutional history of asepsis maximalism. The final letter, from Umezaki to Reinhardt, dated August 1983, thanks him for the historical work: "You have shown what I could not—that this was not inevitable, that choices were made. I needed to know that the science was sound. You made me understand that being sound was not enough."
Umezaki died in January 1984. Reinhardt lived another twelve years, publishing a final reflection on their collaboration in 1990. The correspondence itself was formally accessioned into the Institute's archives in 1995, following Reinhardt's death, as part of the broader Umezaki Papers. Scholars working on the institutional history of serum therapy have since identified the Umezaki-Reinhardt exchange as the hinge point between narrative history and quantified analysis, establishing the pattern that would govern twentieth-century medical historiography in a world without systemic chemical cures.
