The Geneva Sanitary Bureau Vaccination Protocol Manual was the standardized technical reference issued in 1957 by the Geneva Sanitary Bureau to regulate the preparation, monitoring, and deployment of vaccinated livestock used in serum production. It emerged from the recommendations of the Bacillary Congress of Geneva and established uniform procedures for maintaining herds of animals hyperimmunized against bacterial pathogens, particularly those producing protein toxins like the anthrax bacillus.

The manual's existence marked a recognition that serum therapy, still in its early years of systematic clinical use, depended entirely on a reliable biological supply chain. Where chemical antibacterials like the azo drugs could be synthesized in any adequately equipped factory, serum production required living animals, standardized immunization schedules, careful health monitoring, and rigorous collection protocols. The Bureau recognized that inconsistent vaccination practices across nations would fragment serum stock quality and make international serum therapy trials impossible to interpret.

The manual covered vaccination protocols for horses, sheep, and cattle selected for antitoxin production. It specified the choice of immunizing antigens—heat-treated bacterial cultures or purified toxins—and detailed regimens for priming animals, boosting immunity at prescribed intervals, and monitoring antibody titre through specified laboratory methods. It prescribed hygiene standards for housing, feed, and water, and required veterinary certification before any animal entered serum production. The manual also codified the intervals between vaccination and bleeding, the volume of blood that could be safely withdrawn, and the holding temperature and duration before serum separation.

These were not theoretical matters. Poorly timed collections could yield low-titre serum; animals stressed by frequent bleeding produced diminished immune responses; contamination during collection or storage could render entire batches useless or dangerous. The manual's prescriptions reflected the cumulative experience of serum farms across Europe and North America, where managers had gradually discovered through trial and loss what worked and what did not.

National adoption was uneven. France, whose Pasteur Institute had pioneered serum production under Émile Roux and maintained specialized serum farms throughout the postwar period, implemented the protocols rapidly. The Soviet Union, developing its own serum capacity under the direction of bacteriologists in Moscow and Leningrad, published parallel standards in 1958 that followed the Bureau's manual closely, though the Bureau's role was never acknowledged. The United States incorporated the protocols into the regulations of its Public Health Service, and by 1960 most Western nations had adopted or adapted the manual's standards.

Britain's Lister Institute resisted some provisions, particularly the Bureau's specification that vaccinating animals receive only killed bacterial cultures rather than attenuated live strains. The Lister Institute maintained that carefully selected live attenuated strains produced superior antibody responses and had been proven safe through decades of use. A 1961 correspondence between the Institute's director and the Bureau's technical committee, preserved in the Bureau's Geneva archives, shows the disagreement was settled by acknowledging both methods as acceptable where national regulators approved them—effectively an admission that the manual represented consensus rather than settled fact.
