Yeo Koon Sian

Yeo Koon Sian (1909–1987) was a Chinese public health administrator and epidemiological statistician whose systematic documentation of infection outcomes across the Asia-Pacific region established the statistical foundation for coordinated asepsis maximalism in a world without reliable systemic antibacterial treatment. His work, conducted primarily from Singapore and latterly from Hong Kong, made visible the true cost of untreatable bacterial infection at the population level and provided the quantitative evidence that made the Bacillary Congress of Geneva actionable rather than merely aspirational.
Yeo was born in Singapore to a merchant family with ties to both traditional Chinese medicine and British colonial administration. He studied mathematics and medicine at the University of Singapore before training in epidemiology at the London School of Hygiene and Tropical Medicine from 1933 to 1935, where he encountered early sulfonamide chemistry and the emerging recognition that serum therapy and prevention would likely remain the only systemic tools available for bacterial infection. By the early 1940s, working as an assistant to the Colonial Medical Service, Yeo had begun systematically recording hospital infection outcomes across the Malayan peninsula and British Borneo, building paper records during a period when such documentation was fragmentary and inconsistent across colonial territories.
The turning point came in 1952. Yeo attended the Bacillary Congress of Geneva as a delegate from the Malayan medical authority and was asked by Richard Reinhardt of the Institute for the History of Bacteriology to coordinate infection statistics from the Asia-Pacific colonial medical systems. What followed was nearly a decade of painstaking correspondence, standardization meetings, and railway journeys through Southeast Asia, persuading hospital superintendents and district physicians to report their cases in the same form. Yeo designed reporting templates, trained clerks in data collection, and by 1960 had assembled the first statistically coherent picture of infection outcomes across a region spanning millions of people and dozens of institutional traditions.
The results were stark. Mortality from septicaemia and untreated secondary bacterial infection in surgical patients remained between 15 and 40 percent across the region, with variation correlating sharply not with climate or wealth but with the completeness of asepsis maximalism protocol adoption. Childbed fever deaths, running between 3 and 8 per thousand live births, showed the same pattern. Yeo's 1961 regional survey, presented to the Geneva Sanitary Bureau, demonstrated that institutions with systematic post-operative asepsis protocols and dedicated clean wards reported infection rates a third those of hospitals without such infrastructure. The data made the case for capital investment in prevention so clearly that within five years nearly every significant hospital in the region had begun redesign or reconstruction to support asepsis maximalism doctrine.
Yeo's most consequential work, however, was methodological rather than merely descriptive. Working with Paul Kaplan of Harvard and Dorothy Umezaki of the Infectious Disease Research Centre from the mid-1960s onward, Yeo adapted survival analysis and population-level statistical inference to infection outcomes measured over years rather than chemical response measured in days. The Kaplan-Meier method, developed for industrial reliability testing, proved applicable to serum therapy trials and infection prevention protocols when applied to regional datasets. Yeo's role was demonstrating that outcomes measured across entire populations, disaggregated by hospital protocol and patient risk factor, yielded evidence as robust as laboratory measures had been for chemical treatments.
In 1965, Yeo accepted appointment as director of the newly established Asia-Pacific Sanitary Commission, a subsidiary body of the Geneva Sanitary Bureau charged with coordinating asepsis maximalism standards and infection surveillance across the region. The Commission's Statistical Records and Quarterly Reports became the authoritative source for infection outcomes data from Japan to Australia, allowing public health authorities to compare outcomes, identify institutional failures, and justify expenditure on asepsis maximalism infrastructure. Yeo held the position until his retirement in 1982, publishing quarterly through 1987 and building a statistical archive that remains the most complete record of infection outcomes in the region during the peak years of asepsis maximalism implementation.
Yeo never published a monograph or attempted to theorize his work within broader medical history. His intellectual legacy rests on the architecture of data collection itself: the forms, the training methods, the standardized definitions, the quarterly reports that made regional infection patterns visible for the first time. This work was technical, unglamorous, and foundational. The shift from anecdotal ward observation to statistically grounded protocol comparison would have occurred eventually, but Yeo's decade of organizing reluctant hospital administrators and colonial bureaucrats compressed decades into years and made the case for asepsis maximalism not through rhetoric but through quartile comparisons of mortality outcomes.
